DHI hair transplant

DHI (Direct Hair Implantation) is a hair transplant technique in which follicles are implanted with a special tool called a Choi implanter pen, without opening channels first. As in FUE, the follicles are extracted one by one from the donor area; the difference lies in the implantation stage. Dr. Noyan Süalp performs DHI at Dr. Bayer Clinics in Istanbul.

How is DHI performed?

  1. Examination and planning. The hairline, the area to be treated and the estimated number of grafts are determined.
  2. Local anaesthesia. The procedure is performed under local anaesthesia.
  3. Extraction. Follicles are collected one by one from the donor area with micro instruments.
  4. Implantation with the Choi pen. Each graft is loaded into the pen and implanted into the skin in a single movement, with control over its angle, direction and depth.

Features of DHI

  • Because channel opening and implantation happen in one step, grafts spend less time outside the body.
  • It allows precise work in areas that need dense packing and when shaping the hairline.
  • It carries a low risk of damaging existing hair when grafts are placed between them, which is why it is often used in unshaven hair transplants.
  • It takes more time and an experienced team; for very large areas it may be planned together with FUE.

DHI or FUE?

Both techniques have their advantages, and no single technique is equally suitable for everyone. The choice depends on the extent of your hair loss, the condition of your donor area, your hair type and your expectations. You can read more in our article on FUE hair transplant.

Recovery

Recovery after DHI is similar to FUE. Mild swelling and redness are possible in the first days and the scabs usually come off within 7–10 days. Shock loss may occur between weeks 2 and 8. New hair starts to grow after about 3–4 months and the final result usually develops by around 12 months.

Possible risks

Temporary swelling, redness, itching and tenderness are the most common side effects. Infection is rare. Routine tests before the procedure and following the aftercare instructions reduce the risks.

How long does a DHI procedure take?

Usually 6–8 hours, depending on the number of grafts. Because each graft is implanted individually with the Choi pen, DHI can take a little longer than FUE for the same number of grafts.

When will I see the results of DHI?

New hair starts to grow after about 3–4 months, a clear change is visible from month 6 and the final look develops by around 12 months.

Is DHI painful?

The procedure is performed under local anaesthesia. You may feel brief discomfort during the anaesthetic injections. Mild tenderness afterwards is usually managed with painkillers.

Can DHI be done without shaving my head?

In some cases, yes. If the donor area and the number of grafts allow it, DHI can be performed unshaven. This is decided after the examination.
